Cancellations to the Pharmacists and Patient Centered Diabetes Care Certification Training program ONLY prior to this date: If you have not redeemed the enrollment code a refund minus a $75.00 processing fee. If you have redeemed the enrollment code you will be charged a fee of $225.00 whether or not you have completed any portion of the home-study. Event PSSNY’s Registration Cancellation Policy:This policy applies to all events sponsored by PSSNY and regional affiliates. For refunds, all cancellations must be received in writing no later than 48 hours prior to the scheduled event. A $10.00 (per person) processing fee will be deducted from all refunds. Phone cancellations will not be accepted. Cancellations may be received via fax, email or regular mail. It is up to the registrant to confirm receipt of the cancellation request. No refunds will be issued for cancellations received after the deadline. In extenuating circumstances, to be determined solely by PSSNY, a voucher for attendance at another event might be offered. Additional fees may be required.
