Health coaches for our patients with chronic
illnesses and peer counselors for pregnant
women – these members of FamilyCare’s
health care team work individually with
Annual
patients to identify and reduce barriers to
health. For example, a health coach might
meet with a patient and our dietitian to
learn new ways to prepare food and keep
his diabetes under control. A peer counselor
might give practical tips to help with
discomforts of pregnancy. Through funding
from the Benedum Foundation, we developed
a standardized training program for medical
assistants to gain additional knowledge to
become health coaches, and we expanded
our health coach program to offer these
services at our four largest sites. The Strong
Report
Start peer counselor program, which is
coordinated by the American Association
of Birth Centers and funded through the
CMS Innovation Center, allows us to offer
assistance to pregnant women at
two sites. FamilyCare patients,
in partnership with their
primary health care provider,
have easy access to a health
care team that fits each
person’s needs.

New Technology:
Patient Portal
All FamilyCare sites switched to a new
electronic health record and practice
management system in September 2013. We
implemented athenahealth, an innovative,
award-winning, cloud-based system that
includes a patient portal. Through our portal,
patients can request appointments, view
test results and a summary of care, make
payments, and read about health conditions.
Other than a slowdown for a month or so
while we learned the new system, we had
very little disruption in patient care.
Addition of the Children’s Medicine Center
(located on the ground floor of Women and
Children’s Hospital) has been in the works
for a long time and we are excited that this
well-known pediatric practice and teaching
center has become part of FamilyCare.
Our staff has worked with Charleston Area
Medical Center and West Virginia University
Department of Pediatrics to work out all of
the details. The transition went smoothly and
the staff is positive about the changes, which
include a minor facelift and the move from
paper charts to an electronic record. We are
happy to be involved in the education of the
next generation of pediatricians currently in
training at the Children’s Medicine Center.
Madison Site
Safe, modern and efficient
We have completed the building renovations at our Madison
site. Our staff and patients can now enjoy this safe, modern
facility. These renovations have provided us with a workflow
that will allow us to provide care in a more efficient and
pleasant manner to all of our patients. The staff now has
appropriate workspace and modern communication devices.
Our patients are enjoying the additional exam rooms and
appreciate the handicapped parking spaces and automatic
doors located at both our first and second floor entrances.
It is evident that our new windows and new heating
and cooling system are providing us with superior energy
efficiency. We expect that we will see an increase in activity
and productivity at our site in the coming months and we look
forward to many years in our newly-renovated work area.
