RESOLUTION APPROVING ADDENDUM NO.2 TO THE LEASE
AGREEMENT WITH JUDITH CARLESON,TRUSTEE,FOR
CALWORKS OFFICE SPACE AT 715 MALTMAN DRIVE,GRASS
VALLEY,CALIFORNIA
WHEREAS,through Resolution 12-019, the County of Nevada entered into a Master
Lease Agreement and Addendum No. 1 with Judith Carleson, Trustee, for certain property located
at 715 Maltman Drive; and,
WHEREAS,the Agreement expires on June 30, 2014 and both parties desire to exercise
the option to extend the Term of the Agreement in one year increments for a maximum period of
three years; and,
WHEREAS, all other terms and conditions of the Master Lease Agreement and Addendum

remain
in full force and effect.

NOW,THEREFORE,BE IT HEREBY RESOLVED by the Board of Supervisors of the
County of Nevada, State of California, that the Board of Supervisors hereby approves in the form
attached hereto, Addendum No. 2 to the Master Lease Agreement between the County of Nevada
and Judith Carleson, Trustee, for office space located at 715 Maltman Drive, Grass Valley,
California, 95945. The Chair ofthe Board is hereby authorized to execute Addendum No. 2 on
behalf ofthe County of Nevada.

PASSED AND ADOPTED by the Board of Supervisors of the County of Nevada at a regular meeting of
said Board, held on the 13th day of May,2014, by the following vote of said Board:

Extension of Lease Avreement: For good consideration, Landlord and Tenant each agree to
extend the term of said Lease Agreement in one (1) year increments for a maximum period of
three (3) years, commanciny on July 1, 2014, terminating on June 30, 2017. In the event
that Tenant does not provide Landlord with at least 90 days prior Written notice of their
intention to extend, the Lease Agreement Will automatically renew for the next incremental
year with no further documentation. During extended term, it is provided that all other
teams of the Lease Agreement shall continue during this extended term as if set forth
dated
December 13, 2011
This agreement shall be bindinv upon and shall inure to the benefit of the parties, their
successors, assigns and personal representatives.
The foregoing terms and conditions are hereby agreed to, and the undersigned acknowledge receipt of a copy of this document.
