Vendor Booths Pricing: Corner booths are an additional $150.00. (Availability limited.) 50% Non-Refundable Deposit
due with this application. Booth size approximately 10’ x 10’. Drapery (8’ back drape & 3’ side rail) is included. Please
use your own signage. In order to take advantage of the member rate, your 2015 member dues must be paid by February
3, 2015 otherwise, you will need to pay the non-member rate.

Sales Tax Certificate (Contact 518-485-2889 or www.tax.ny.gov – Register as a Sales Tax Vendor) Please include a

Liability Insurance - ALL VENDORS must have liability insurance (see Sample Certificate) naming the Western New
York State Nursery and Landscape Association, Inc. (WNYSNLA, Inc) DBA PLANT WNY and the Erie County
Agricultural Society, Inc. as additional insured. PLEASE have it written as the sample indicates and send as soon as
possible.
I certify the information stated in this application is complete and true, to the best of my knowledge. It is understood that
this is an application only and is not a lease agreement. A lease agreement will follow acceptance of this
application/deposit. "Plantasia" reserves the right to reject an application and will return deposit if application does not
meet with approval. All deposits, once accepted, are non-refundable.
